Each Autumn Health Care Inc., facility is unique in the specialized services they provide. Whether for long or short-term care, Autumn Health Care, Inc. facilities pride themselves in being the provider of choice in the community.

Autumn Health Care Inc.’s first skilled nursing facility was a 48-bed skilled nursing facility located in Newark Ohio. Autumn Healthcare of Newark is unique because it also offers an additional ten-unit senior living complex.

The second facility acquired by Autumn Health Care Inc. was located in Coshocton Ohio. In 2004, the 74-bed Autumn Healthcare of Coshocton, was relocated to a newly built state of the art facility providing residents with several amenities.

The third facility established by Autumn Health Care, Inc. is the organization’s only assisted living facility. Autumn Healthcare of Mt Vernon offers a unique homelike setting for residents requiring assistance with everyday tasks or those individuals needing Alzheimer’s and dementia care.

Autumn Healthcare of Zanesville was the fourth location purchased by Autumn Health Care, Inc. Our new state-of-the-art facility opened in January 2008. Autumn Health Care of Zanesville offers short-term rehabilitation, long-term care and Alzheimer’s and Dementia care.

Autumn Healthcare of Cambridge was the fifth facility under Autumn Health Care Inc.’s direction.. Autumn Health Care constructed a brand new state-of-the-art nursing and rehabilitation center that opened it’s doors in December 2006.

Autumn Healthcare of Somerset was the sixth facility acquired by Autumn Health Care Inc.  This 80-bed facility offers short-term rehabilitation, long-term care and respite care.

Autumn at the Lake located in Thornville will be the seventh facility under the direction of Autumn Health Care.  This facility will have a nautical Cape-Cod style facility specializing in short-term rehabilitation and long-term care.  Completion of this exciting project is early 2008.
